Paranoia

Stage 1
You begin to be suspicious and unsure of those around you. You have disadvantage on all Social checks.
Stage 2
Everyone wants to further their own motives, even if it isn't clear. Treat social encounters as if your character's disposition were one stage worse.
Stage 3
It's very easy for you to become distracted in crowds. In any settlement larger than a hamlet, you are permanently shaken. Further, at the start of each combat round, you must choose a number of enemies equal to 2 plus your Wisdom modifier. All other enemies have advantage to attack you, as you are too focused on the chosen ones.
Stage 4
Everyone is out to get you, and are actively working against you. Your disposition with any other being cannot be higher than hostile, though you do not need to immediately attack them if it is not in your interests. You're not crazy, after all.
